Dealing with patients that may have potential cancer requires a “one-stop” approach. Patients are seen on the day in clinic having a full evaluation by the surgeon and subsequently any lumps in the neck can be looked at under ultrasound guidance and appropriate needle biopsies undertaken by specialist consultant radiologists.

State-of-the-art equipment

State of the art equipment allowing for endoscopic swallow evaluation. In addition EMG (electromyographic) recordings and X-ray evaluation of swallow will be the standard of evaluation for patients with dysphagia. Close collaboration involved in managing patients in his NHS clinics is being replicated.

Pioneering a multi-disciplinary approach

Multi-disciplinary swallowing clinic evaluation is in its infancy in the United Kingdom. Mr Ghufoor is pioneering a multi-disciplinary approach working with colleagues in gastroenterology, swallow therapists as other core members involved clinic evaluation of patients with dysfunctional swallow.

EAR BLOCKAGE

There are many causes for blockage of the ears or hearing loss some of which can be related to wax build-up in the ear and occasionally fluid or other infections within the ear canal or eardrum. These can be carefully inspected and dealt with under microscope assisted magnification and appropriate specialist ear instruments. This can alleviate the issues almost immediately. Specialist hearing testing services are also available in many of the clinics.

THROAT INFECTIONS/PAIN

There are many causes for pain in the throat or in the neck. Some of these can be related to the tonsils or structures in the back of the throat. Others may well be related to the muscles of the neck as they are attached to the throat. Determining the cause can allow for the appropriate treatment and the vast majority of patients seen in the clinic can be reassured that there are no serious or sinister causes for their symptoms.

NOSE BLOCK AND NOSE BLEED

These are common problems many of which are simply related to local problems in the nose. Careful evaluation in the clinic with possible endoscopic evaluation identifies the vast majority of causes that can account for these symptoms. These can usually be managed in the clinic but occasionally may require treatment particularly for troublesome nosebleeds that may require outpatient local anaesthetic cautery. These are conducted as a one-stop event in the clinic.

Mr Ghufoor is part of the Endocrine Multi-Disciplinary Team based at St Bartholomew’s Hospital. He deals with complex and difficult thyroid/parathyroid cases. He runs a regular combined theatre list with endocrine physicians & cardiothoracic surgeons where patients who have large thyroid glands extending from the neck into the chest are dealt with in a safe environment. The key members of the team involved in managing these patients are integral to the thyroid/parathyroid specialist clinic that Mr Ghufoor runs in Harley Street and The London Bridge Hospitals and the operative team working in central London.

Salivary gland disorders can arise from swellings or growths predominantly in the parotid and submandibular glands. Usually these are benign problems but do require surgical treatment. Careful ultrasound scan evaluation can help to determine the best modality of treatment and to identify the nature of the problem. Occasionally patients may have some problems relating to the flow of saliva from the glands into the mouth and these can be dealt with through minimal access techniques.
